3. Sample Letter Requesting an Accommodation Plan

A person with a disability is defined as someone who: 1) has a physical or mental impairment that significantly limits a major life activity; 2) has a record of such an impairment; or 3) is considered to have such an impairment under Section 504 of the Rehabilitation Act.

A plan for how a school will support a student with a disability and remove obstacles to learning is how Section 504 manifests in schools. Giving the student equal access to the school and its activities is the aim of a 504 plan.

A parent can ask for a 504 plan by contacting the principal or the school's 504 coordinators. A written request is required. After that, a school will meet with a parent to determine whether the child qualifies and what supports are best.

When coming to that conclusion and formulating a plan for student accommodations, the school team will use data from several sources.
